Getting Ready for Your First Doctor's Appointment
Choosing a new healthcare provider can be overwhelming, which is why we strive to make the process as smooth as possible.
1) To establish care at Brookville, please call our office and schedule an appointment with one of our physicians.
2) Please sign and fax the release of records forms before your appointment so that we can fully understand your child's health history.
We will review your child's medical history before your visit, allowing us to create a tailored care plan that meets your child's unique needs.
